Dollar General Literacy Foundation: The Dollar General Literacy Foundation has grant opportunities for all age groups, and all types of communities.  They currently support literacy programs in 35 states.

Program reporting is required upon receipt of the grant.

Verizon Foundation: The Verizon Foundation aims to improve lives in "literacy, knowledge and readiness for the 21st Century."  To receive a grant, an organization must be prepared to track and measure program outcomes for reporting.
